Preschool Requirements

ITEMS REQUIRED TO REGISTER YOUR CHILD FOR PRESCHOOL:

1. Child Enrollment Form
2. Birth Certificate
3. Immunization Records*
4. Child Medical Statement**
5. Emergency Medical Authorization
6. Text Messaging and E-Mail Permission Form
7. Information Regarding Legal Custody
8. Must Be Toilet Trained 

*By State Law, the Immunization record is due by the first day of attendance. If a student from the previous year, and the immunization record is up to date, we can use that record.

**The Child Medical Statement MUST be signed by the doctor and is due no later than 30 days after attendance. If a student from the previous year, and the Medical Statement is dated within one year, we can use that record.
